Pre-Entrance University Physicals |

All freshman, transfer and first year student-athletes must obtain a physical from his/her personal physician prior to entering the university. CCSU’s physician cannot perform this pre-entrance physical. It. is mandatory that the university pre-entrance physical be completed before the athlete arrives on campus. Pre-entrance physical forms may be obtained by calling Health Service at 860-832-1926.
ImmunizationsAll student-athletes must have evidence that their measles, mumps, and rubella vaccines are current. This information must be recorded on the physical examination form provided by CCSU Health Service. Athletes who cannot furnish this information may receive their MMR shots for a nominal fee at Health Service. Athletes will not be given a sports physical without record of immunizations. Also, athletes are required to have an up to date tetanus booster. Student-athletes assigned to a dormitory must be vaccinated for meningitis. Meningitis and Hepatitis B vaccination is strongly recommended for all student-athletes.
Sport Physicals q Freshman, Transfers, and First Year Participants - CCSU’s physician performs a sports physical. The sport physical will not be performed unless the athlete has on file a university pre-entrance physical that includes record of all immunizations. . Sports physicals require an appointment with Health Service, 860-832-1926.
q Veteran (returning varsity athletes) - will be screened for blood pressure and pulse each season. Also, a medical history update must be filled out and all vaccinations must be current including Tetanus boosters.
Cardiac Exams Student-athletes who have had cardiac exams for previous conditions will be required to submit a copy of those records to CCSU’s Health Service prior to being allowed to participate in intercollegiate athletics, including preseason strength and conditioning programs.
Recent Surgery or Physical Therapy All student-athletes who have had surgery within the past one-year must submit a physician’s note stating that they are cleared for full participation in their particular sport on file in Health Service and in the Athletic Training Department. Should the athlete develop a significant injury or illness after his/her physical exam, but before the first practice session, then the athlete must present a note from a qualified physician clearing them for intercollegiate athletic participation. It is the student-athlete’s responsibility to request the note from the physician and present it to the athletic training staff and/or Health Service.
