Software Request Form Instructions
1. Open the PDF document (Request Form) by clicking the link at the bottom of this page. Please read through all of these instructions before opening the form.
2. Fill out the form with all the necessary information. Use tab or click to move from field to field. The Justification field is very important!
3. After filling out the information, use the Save As command to save the request form (put the software name in the title). Email the saved form to Lisa Washko (firstname.lastname@example.org) as an attachment.
If you have more than one request, please submit multiple forms (one for each request).

The request form is NOT just for new/upgrade software purchases. It must also be used for any specific software that you do not see on the list of software currently installed in computer labs and classrooms on campus (see link at the bottom of this page). If your department has purchased software or you want to have freeware installed in a specific room or rooms supported by the IT Department, for the Fall semester, a request form must be submitted by the deadline. This is necessary for testing, planning and prioritization purposes.
All requests will be reviewed by a subcommittee of the Information Technology Committee (ITC). The requests will be prioritized taking into consideration budgeting restraints and justifications; not all requests will be funded.
You will be notified with the status of your request, as determined by the ITC, by the end of the Spring semester.
