If you have an appointment you will see your scheduled appointment with applicable forms related to your appointment. If you are uploading forms please follow the instructions pertaining to UPLOADING DOCUMENTS. For ALL in person appointments you will need to use this portal to self check in for your appointment - no sooner than 1/2 hour prior to your scheduled appointment.
